Overview
Position Summary
The Lincoln County Behavioral Health division is looking for a Community Mental Health Nurse to help provide necessary services to our community. This person will perform a wide variety of community based, professional mental health nursing duties, which includes direct patient care and nursing case management. They will also promote physical and emotional health, address health concerns of patients, educate clients on various health care issues and provide information on health choices.
This role is also responsible for maintaining records and files according to all applicable County policies and procedures and makes reports as required. Maintaining client contact to comply with division productivity standards.
$40.19 - $54.02 per hour

Community Health Nurses will follow the six standards of practice:
- Assessment – By means of home visits and/or office-based visits, conduct patient interviews and chart review, work with clients to develop a plan of treatment. Documents assessment in terms that are understandable to all involved in the care process. Performs emergency general mental health assessment, referral and counseling duties, including crisis intervention, as required. May provide direct observation of clients taking medication or observe clients filling pill minders.
- Diagnosis and Priorities – Analysis of the assessment to derive a mental health diagnosis and service priorities. Documents diagnoses or concerns in a manner that facilitates the client’s involvement in developing a plan of care.
- Outcomes identification – Based on the diagnosis the CMHN will identify outcomes that are culturally relevant, health literate, based in evidence and attainable through local resources and policies. Document expected outcomes and measurable objectives.
- Planning – Develop a plan that is client-centered and reflects best practices by identifying strategies, action plans and alternatives to meeting expected outcomes. Document the plan using health literate and culturally competent language that can be understood by care team and individual being served.
- Implementation – The CMHN oversees the implementation of the plan through treatment, case management, referrals to community partners, administration of long acting injectables, collaboration with community resources, health promotion and education. Documents activities and status of care coordination.
- Evaluation – Apply on-going progress assessment to revise plans, interventions and activities as appropriate for ultimate health outcomes for the individual. Documents changes to enhance effectiveness of interventions.
- Attend and participate in team meetings, trainings, and activities to further the mental health of the community.
- Participate as a member of the interdisciplinary treatment team for identified clients as indicated.
- Provide staff training relative to best practices, as assigned.
Education
Associate's degree in nursing from an accredited school of nursing.
Experience
Two years of experience in a mental health setting. Lived experience with mental health may substitute for years of experience.
Driver's License
Must possess a valid Oregon Driver's License within 30 days of hire.
Other Licenses
Must possess a valid license to practice in the State of Oregon as a Registered Professional Nurse as defined by statutes or administrative rules, which include but are not limited to ORS 678.010 to 678.360 at the time of hire. The employee is required to maintain current and active licensures and certifications, as applicable, and to provide the appropriate verifications at the time of renewals.
Desirable Qualifications
Bachelor's degree in nursing preferred. Bilingual and/or bicultural competencies for the Latino population are preferred.
