Job Summary

We are a friendly team looking for an enthusiastic Practice Nurse to join our team. You will undertake clinical practice using expert knowledge and clinical skills to deliver exceptional care to people accessing our primary care services. An NMC registration is mandatory.

Newport Health Centre is based centrally on the Isle of Wight operating across two sites;
Carisbrooke practice and Dower House, Pyle Street. We support over 24,000 patients with a diverse, multi-disciplinary team including GP Partners, Advanced Nurse Practitioners, Paramedics, MSK Practitioners, Mental Health Practitioners, Practice Nurses, HCAs, Chronic Pain Coach, Care Coordinators, a Social Prescriber and more. We are part of the Central and West Primary Care Network (PCN).

Job Description

Main Responsibilities

  • Leadership:
    Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model. Promote and work to the practice’s Vision and Values.
  • Clinical practice:
    Assess, plan, develop, implement, provide and evaluate treatment programmes including, but not limited to chronic disease management; health promotion and well-being; management of long-term conditions; vaccination and immunisation programmes; wound care; minor illnesses; and opportunistic care.
  • Delivering a quality service:
    Produce accurate records; manage workload; deliver care in line with NSF, NICE guidelines and evidence-based care; implement quality governance systems; plan nursing appointments across sites and adjust for seasonal needs; promote evidence-based practice.
  • Communication and Relationships:
    Demonstrate sensitive communication; act as advocate; manage confidential information; ensure effective communication tailored to individuals; uphold professional reputation and confidentiality under Data Protection Act.
  • Analysis and Judgement:
    Ensure competence; manage risk; monitor safety; apply infection-control measures; participate in local implementation strategies.
  • Planning and Organising:
    Prioritise workload; be flexible to take on other duties as needed.
  • Service and Policy Development:
    Be aware of practice policies/procedures; develop clinical standard operating procedures where required.
  • Personal Development:
    Take responsibility for mandatory training; undertake additional development at appraisal.
  • Information System Use:
    Use systems such as Systm One, Team Net, Word, Excel, Access, etc.
  • Autonomy:
    Work on own initiative; manage workload and adjust priorities; supervise discussions and refer issues to line manager as needed.

Person Specification

Essential Qualifications

  • Maths and English GCSE or equivalent experience
  • Registered nurse with NMC
  • Relevant nursing/health degree

Essential Skills

  • Good interpersonal skills with diverse groups
  • Self-directed practitioner; strong IT skills; effective time management
  • Ability to work as part of a team and autonomously; problem solving and attention to detail

Desirable Skills

  • Clinical skills such as cervical cytology, immunisation and vaccination, wound management, minor illness, contraception
Personal Qualities
  • Planning and organising; flexible and cooperative; motivated and proactive
  • Forward thinker; integrity and loyalty; resilient; able to work under pressure
Experience
  • Desirable: experience in nurse-led management of long-term conditions, GP practice experience, quality initiatives, public health promotion, clinical governance, patient group directions
Other
  • Disclosure Barring Service (DBS) check
  • Willingness to travel to other sites
  • Flexibility to work outside core hours; maintains confidentiality; full UK driving licence
