As a Full‑Time Registered Nurse and Start of Care (SOC) RN with Personal‑Touch Home Care, you'll be integral in planning, organizing, and delivering home care services. We're seeking experienced nurses with a passion for community health education and engagement. Utilizing community resources, you'll tailor services to meet the unique needs of individuals and families within their homes and communities.

• Conduct comprehensive assessments of patients and families to determine home care needs.
• Provide thorough physical assessments and medical histories.
• Continuously reassess patient needs and adjust care plans accordingly.
• Collaborate with physicians to develop personalized care plans.
• Involve patients and families in the planning process.
• Implement preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Offer counseling and health education to patients and families.
• Identify discharge planning needs and implement plans prior to discharge.
• Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.

• Current licensure in the State of Virginia.
• CPR certification and a valid driver's license.
• Preferably, a minimum of one year of recent acute or long‑term care experience.
• Bachelor's degree preferred, with at least one year of experience in home care.
• While management experience is not required, responsibility for supervising Home Health Aides and Personal Care Aides.
• Strong observation, verbal, and written communication skills.
• Excellent problem‑solving abilities and basic math skills.
