Overview
The Licensed Practical Nurse works under the supervision of the Care Manager to assist Physicians/Registered Nurses in the care of clients in their homes by providing comprehensive skilled nursing services directed toward maximum rehabilitation to achieve established goals and objectives. Demonstrates knowledge of Medicare, Medicaid and insurance requirements, and agency policies and procedures. Provides age-appropriate care, assesses data reflective of the client e2 9s status and current requirements, and follows departmental policy and procedure guides.
The LPN may be required to work every other weekend depending upon client census. Supplements the nursing care needs of the client as provided by the Registered Nurse.

- Assist physicians/registered nurses in the care of clients in their homes by providing comprehensive skilled nursing services directed toward maximum rehabilitation to achieve established goals and objectives.
- Provide comprehensive skilled nursing services to assigned patients in the caseload; remain knowledgeable about Medicare, Medicaid, and commercial insurance requirements and the scope of service.
- Implement and evaluate plan of care, treatments and procedures that contribute to physiological and psychosocial outcomes; collaborate with the primary RN in providing quality patient care per the Plan of Care; support nursing care needs as directed by the RN or physician; assist with specialized procedures as needed.
- Build trusting, collaborative relationships with a multi-disciplinary team to support patient care needs; actively participate in multi-disciplinary team meetings as assigned.
- Prepare clinical and progress notes documenting outcomes of interventions; accurately complete all required clinical documentation per RHC standards, including timely updating of patient notes and verbal orders.
- Program Graduate, Licensed Practical Nurse (Required)
- Nursing experience in acute care or home care setting (Preferred)
- Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days (Required)
- Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job
- Valid Driver's License Required
