Overview
The Hospice RN On-Call Nurse (7 ON/ 7 OFF schedule) will provide On Call services to support the Hospice department after hours patient needs. The Hospice RN On-Call schedule begins on Monday afternoon at 4:30 pm and ends on Saturday morning at 8:00 am. Your coverage area is defined as the entire area serviced by the Hospice Department. This position is a 7 day on and 7 day off schedule.

- Completes an initial and comprehensive assessment of patient and family to determine hospice needs.
- Provides a complete physical assessment of history of current and previous illness(s) as well as pain and symptom control.
- Provides professional nursing by utilizing all elements of the nursing process and follows the State Nurse Practice Act.
- Assesses and evaluates patient's status by:
- Writing and initiating POC
- Regularly re-evaluating patient and family/caregiver needs
- Revising the POC as necessary
- Initiates the POC and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a PC that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions in concert with the IDT group. Includes patients and family in the planning process.
- Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Provides health care instructions to the patient/family as appropriate per assessment and plan.
- Assists the patient in activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.
- Works the Hospice RN On-Call after hours work schedule as defined above.
- Provides death visits, as necessary.
- Graduate of an accredited school of nursing.
- Two years' experience of recent acute care experience in an institutional setting and one year of recent experience in a home care setting, preferred.
- Current licensure in State of Virginia, CPR certification and a valid VA Driver's License.
- Prefer bachelor's degree with one year of Hospice experience.
- Management experience not required. Responsible for supervising Hospice aides and Hospice LPN's.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, hospice nursing skills per competency skills checklist.
