We are seeking a qualified Full-Time Hospice Registered Nurse for the Peninsula Community. This office is located in Chesapeake, VA and provides service to the cities in the Tidewater area that includes Hampton, Newport News, Poquoson, York County, Williamsburg and surrounding areas.

Qualifications
  • Licensed Registered Nurse with current valid license to practice in Virginia
  • 2 Years Hospice Experience Required with OASIS Experience Preferred
  • Knowledgeable of State and Federal regulations
  • Strong computer skills required; familiarity with Netsmart preferred
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills
Job Summary

The Hospice Registered Nurse plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Essential Job Functions / Responsibilities
  • Completes an initial and comprehensive assessment of patient and family to determine hospice needs.
  • Regularly re-evaluates patient and family/caregiver needs.
  • Develops a plan of care that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions in concert with the Interdisciplinary group.
  • Participates in on-call duties as defined by on-call policy.
  • Attends death visits.
  • Performs chart reviews, prepares interdisciplinary meeting notes.
