Overview
Works under the supervision of the Nurse Manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients on an intermittent visiting basis. The RN may work in a home care type setting and in roles within Riverside Health System. The RN uses communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial, and cultural diversity.
Responsible for supervising and delegating non-professional personnel to ensure optimum patient care. Must demonstrate competence in addressing age-related needs of the patient populations served. Works with leadership and a multidisciplinary team to provide a comprehensive range of services in accordance with Riverside Health System standards and facility goals and strategic plans.
- Assess, plan, implement, and evaluate the total nursing care of assigned patients on an intermittent visiting basis. Apply standards of practice as they relate to the specialty or patient population served. Develop initial and ongoing comprehensive assessments, including OASIS assessments at appropriate points.
- Supervise and delegate non-professional staff to ensure optimum patient care. Recognize and respond to emergency situations. Evaluate needs for support and use resources to meet patient care needs. Supervise LPN/LVN and Home Health Aides.
- Complete all required clinical documentation per RHS standards, including locking patient notes and verbal orders, and completing certifications and recertifications after multidisciplinary team meetings. Complete required documentation no later than the last day of the month to ensure timely billing.
- Implement or initiate a plan of care, provide services requiring specialized nursing skills, counsel the client and family, coordinate services, inform the physician and staff of changes in the client's needs, and plan for discharge. Initiate preventive and rehabilitative nursing procedures to support physiological and psychosocial outcomes and stability.
- Participate in case conferences. Provide education to client and family. Participate in training and support of all team members, volunteers and students as needed.
- Registered Nurse (Required)
- Bachelor’s Degree in Nursing (Preferred)
- Staff Nurse (Preferred)
- Home Care (Preferred)
- Registered Nurse (RN) – State Department of Health Professions (Required)
- CPR/BLS Certification – AHA/ARC/ASHI (Required; within 30 days)
- Driver's License (Required)
