A leading health provider in Newport News, Virginia, seeks a Lead Hospice GIP Nurse to guide patients and families during critical moments. This role requires clinical judgment and leadership, coordinating transitions between care settings. Candidates should have a nursing degree and a mix of nursing and home health experience. In this position, you'll build relationships with care teams to provide seamless patient care and exceptional service while remaining knowledgeable about insurance and Medicare requirements.

You'll report to central management for guidance and policy development.
