The Nurse Practitioner functions independently as healthcare provider in a primary care setting under the indirect supervision of a supervising physician. The Nurse Practitioner works collaboratively with the physicians in providing multidisciplinary care for adult and geriatric patient requiring services in an outpatient setting. The Nurse Practitioner is responsible for providing comprehensive care to patients with acute or chronic medical conditions across all ages in outpatient setting that may include tele-health medicine.

Primary responsibilities include identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and Annual Medicare Advantage Visits (AMAV) and execution of guided processes to complete AWV and AMAV. The Nurse Practitioner demonstrates a high degree of clinical expertise in working with patients with chronic illnesses commonly encountered in the adult and geriatric patient population. He/she is responsible for assessment, diagnosis, education, health promotion, and care coordination for patients with acute and chronic health needs.

The Nurse Practitioner demonstrates an advanced level of medical clinical knowledge, communication and interdisciplinary collaboration, professionalism and systems-based practice. In addition, shows advanced knowledge in Risk Adjustment programs and HCC coding and documentation guidelines. The Nurse Practitioner projects an image of professionalism in communication, appearance, and conduct. The Nurse Practitioner supports the mission and vision of the Hoag. He/She will provide care within his/her scope of practice as outlined by applicable state law, licensing, regulations, institutional policy, standardized procedures, and practice agreements.

5. Develops, implements, evaluates and alters the plan of care as needed.

6. Manages general medical and surgical conditions based on knowledge of the indications, contraindications, side effects, interactions and adverse reactions of pharmacologic agents and other relevant treatment modalities.

7. Counsels and educates patients and families.

8. Demonstrates proficiency in procedural skills pertinent to practice area.

9. Maintains accurate, complete, concise and timely documentation in the electronic medical record.

10. Applies knowledge of professional nursing theory, clinical practice standards, medical practice guidelines and scope of Nurse Practitioner roles.
