Overview
The Program Manager for Population Health is responsible for leading, coordinating, and optimizing value‑based care initiatives that improve quality outcomes across Hoag Clinic. This role requires a deep clinical understanding of ambulatory workflows, care management processes, and operational practices to effectively partner with providers, medical assistants, nurses, and operational leaders. The ideal candidate possesses strong expertise in HEDIS measures, CMS value‑based programs, and quality performance methodologies, with the ability to translate complex regulatory requirements into actionable strategies.
Responsibilities- Strong clinical understanding of in-clinic workflows, care coordination processes, and clinic operations
- Demonstrated experience working with HEDIS measures, CMS value‑based programs, and quality performance improvement
- Proven ability to interpret regulatory and payer requirements and translate them into actionable operational strategies
- Experience leading or overseeing teams responsible for medical record retrieval, documentation validation, or gap‑closure support
- Ability to collaborate with operational leaders to assess needs and implement effective tools, workflows, education, and resources
- Ability to monitor performance dashboards, identify trends, and drive data‑informed decision‑making
- Exceptional attention to detail and organizational skills, with the ability to manage multiple high‑priority initiatives simultaneously
- Excellent communication and relationship‑building skills across clinical, operational, and administrative teams
- Experience driving cross‑functional initiatives in a fast‑paced, outcomes‑oriented environment
- Proficiency with population health platforms, EMRs, and quality reporting tools (e.g., Epic, health plan portals, risk & quality)
- Education and Experience
- Required:
3 to 5 years of experience with application support of physician practice systems - Bachelor of Science degree in Computer Science, Business, Healthcare or related field or equivalent years of work experience
- Preferred: Certification(s) in Epic Ambulatory modules
- Database administrator experience
- Experience with CMS, HEDIS, Population Health and Quality Improvement programs
