• Under the direction of a registered nurse or physician and under assigned administrative supervision, provide care within the scope of the Vocational Nurse Practice Act and regulatory standards.
• Assist in the assessment, planning, implementation and evaluation of patient care.
• Follow patient identification procedures, provide appropriate information for all patient procedures, and collect and handle specimens.
• Assess and implement prescribed interventions for the patient.
• Implement and document discharge plan for each patient.
• Recognize that ethical/legal/medical dilemmas exist in the nursing practice and confer with appropriate resource personnel when making ethical nursing decisions.
• Participate in maintaining a safe patient care environment.
• Assist with the care and maintenance of department equipment.
• Complete and accurate charting of patient records and record keeping of equipment and quality measures.
• Collaborate with other disciplines of the health care team to ensure the delivery of care across the continuum, providing timely, safe, and quality care.
• Responsible for providing a safe, HIPAA compliant environment for the patient and staff.
• Perform other duties as assigned.

Provide care in both the home health and hospice setting. Assignments will be throughout Orange County, patient care territories, and may change daily.

• High School diploma or equivalent.
• One or more years of experience in a healthcare care facility (Specialty Clinics can hire a new grad LVN with no experience).
• Basic keyboarding skills and knowledge of Microsoft Office Suite, including Outlook, Word and Excel preferred.
• Basic knowledge of medical terminology preferred.
• Experience with Electronic Medical Records preferred.
• Hospice:
  Community health/hospice or medical/surgical experience preferred. Understands philosophy of hospice concept and needs of the terminally ill. Must be a licensed driver with an insured automobile in good working order.

Current LVN License to practice in the State of California.

Current BLS Certification.

Phlebotomy Certification.
