- Communicate findings as needed to the clients’ primary care physician and/or specialists.
- Triage cases by reviewing alerts and determining next best actions.
- Document communication with client and caregiver timely and accurately, utilizing interviewing skills to ensure maximum engagement and to discern client health status and health needs based on key questions and conversations.
- Educate client, caregiver, and family as appropriate and provide information on available community resources.
- Apply clinical judgment to the incorporation of strategies designed to reduce risk factors and barriers, address complex health and social indicators which impact care planning, and resolve client issues.
- Review client needs/referrals as identified by caregivers and others, ensuring appropriate follow up occurs.
- Management may assign or reassign duties and responsibilities to this job at any time that are not listed above.
- Organized, flexible and able to multitask.
- Ability to work independently and as part of a broader care team as the situation requires.
- Excellent communication skills – both verbal and written.
- Strong problem‑solving skills.
- Ability to leverage community resources to benefit clients.
- State of Georgia or multistate RN licensure.
- One (1) year of experience working in hospital, LTC or home care setting.
- Strong preference for experience with elder population in a home health care, hospice/palliative or facility setting.
- Conducts performance reviews.
- Trains other associates.
- Directs work of other associates.
