Job Summary
We have a new and exciting opportunity for a Clinical Case Manager to join our Clinical Case Management Service (CCMS). This is a hybrid role that requires a driver with access to a personal vehicle and the willingness to travel across the North East and North Cumbria. The service works closely with the North East and North Cumbria Integrated Care Board (IBC) Complex Case Management team to support the discharge of people from acute secondary care inpatient beds or private hospital beds into alternative community settings, assist in the repatriation of people back to the NENC region for those currently residing out of area, and provide assessments, discharge planning support and 12 weeks post‑transition support.
Key Responsibilities- Manage the needs of service users, carers and partner services to ensure delivery meets all relevant quality standards and is person‑centred.
- Manage a team of up to four staff to ensure effective deployment of resources and achievement of agreed outcomes and targets.
- Coach and mentor student nurses and newly qualified nurses.
- Provide auditing of the service on a regular basis for senior management and governance teams.
- Undertake any reasonable duties or responsibilities to meet the needs of the organisation.
- Maintain positive relationships with ICB Case Managers, Local Authority Placement Coordinators and NHS Trust Bed Coordinators.
- Carry out assessments, facilitate discharge plans and create/monitor the 12‑week transition plan when a service user moves into new accommodation.
- Provide information for funding panels to support effective move‑on for individuals.
- Registered Nurse or Registered Allied Professional.
- Significant post‑registration experience.
- Experience working with people with complex mental health needs, autism and learning disabilities.
- Demonstrable experience of meeting agreed performance targets in a service demonstrating clinical outcomes.
- Experience with common mental health problems (e.g., anxiety or depression).
- Robust risk‑management experience.
- Advanced understanding of complex mental health conditions and behaviours.
- Knowledge of local and national policies on mental health service provision and relevant legislation.
- Salary: £36,046.35 to £42,204.37 per year.
- Permanent, full‑time contract (40 hours per week).
