Social Care Practitioner (Adult Social Care - West team)

West Berkshire Council is seeking a Social Care Practitioner to join the Adult Social Care team, reporting to the Team Leader. This role offers a supportive and collaborative environment for delivering essential social care services to the local community.

• Work collaboratively with individuals to assess their social care needs and provide appropriate support and interventions.
• Conduct assessments and reviews of care packages, ensuring individuals receive the necessary support to maintain independence and well‑being.
• Build positive relationships with service users, their families, and other professionals to ensure holistic care delivery.
• Provide information, advice, and guidance to individuals regarding available services and support options.
• Advocate for the rights and needs of individuals, promoting independence and inclusion within the community.
• Coordinate care and support services in partnership with external agencies and providers.
• Contribute to continuous improvement through participation in team meetings, supervision, and professional development activities.

• Relevant social care qualification.
• Experience working in a social care environment, ideally within an adult setting.
• Sound knowledge of social care legislation, policies, and best practices.
• Strong assessment and care‑planning skills, able to identify and address individual needs.
• Excellent communication and interpersonal skills, capable of building rapport with service users and multidisciplinary teams.
• Empathy, compassion, and a person‑centred approach to care delivery.
• Ability to work independently, manage a caseload effectively, and prioritise tasks.
• Knowledge of safeguarding principles and ability to identify and respond to risks.
