To give expert direction and leadership having overall responsibility for the management of the children and young people’s continuing care process within the community children’s nursing service and for the governance arrangements to ensure safe and effective care in line with the prudent healthcare agenda
To work with the team managers within the community children's nursing service (CCNS), other professionals and partner agencies and provide professional and expert advice and training in regard to the children and young peoples continuing care process.
To support the Senior Nurse and CCN Service by participating in strategic development of the continuing care service and actively contribute to the development of nursing clinical standards, policies and procedures to ensure that implementation of continuing care is sustainable and meets children and young people’s assessed needs.
Main duties of the job- Assess eligibility for continuing care through comprehensive, multi agency assessments.
- Develop and commission care packages tailored to the child/young persons complex health and social needs.
- Co ordinate across agencies ( health, social care, education) to ensure integrated support.
- Provide clinical leadership and contribute to service development and policy improvements.
- Ensure Safeguarding and compliance with the National Framework and local governance.
- Handle complaints and appeals related to continuing care provision.
