Wound Ostomy RN - Home Health
Providence Home and Community Care (HCC) is hiring a Wound Ostomy RN for home health services in Oregon, supporting communities in Hillsboro, Newburg, Tigard, and surrounding areas. This is a full‑time, day shift position.
Responsibilities:
- Provide direct and indirect quality nursing care to patients of all ages, following the nursing process and the Nurse Practice Act.
- Deliver wound, ostomy, and continence care in patients’ homes, ensuring individualized treatment plans.
- Collaborate with interdisciplinary teams to coordinate assessments and interventions.
- Maintain accurate patient records and documentation in electronic health records.
- Participate in professional development and continuous education related to wound and ostomy care.
Required Qualifications:
- Bachelor of Science in Nursing (BSN) from an accredited nursing school.
- Completion of an accredited course in Wound, Ostomy, and Continence.
- National Provider BLS certification from the American Heart Association within 30 days of hire.
- Oregon Registered Nurse license upon hire.
- Certification as an Enterostomal Therapist by the Wound, Ostomy & Continence Nursing Certification Board within 90 days of hire.
- Minimum two years of nursing experience, including wound and ostomy care.
Preferred Qualifications:
- One year of home care experience (prior or upon certification as an Enterostomal Therapist).

Location: Mortuary Building – West Portland, OR 3601 SW Murray Blvd.
Salary: $51.72 – $72.14 per hour, plus potential shift differentials, on‑call, overtime, and other incentives.
