Social Worker Director

About the Role

Newark Rehabilitation is seeking a Social Worker to serve as our Director of Social Services. This role is responsible for planning, organizing, implementing, and overseeing the Social Services Department to ensure that the emotional and social needs of our residents are consistently met in accordance with federal, state, and local regulations.

• Plan, develop, organize, implement, evaluate, and direct social service programs.
• Stay current with federal and state regulations and recommend updates to policies and procedures.
• Participate in discharge planning, care plan development, and resident assessments.
• Coordinate ancillary resident services (optometrist, audiologist, psychologist, dentist, etc.).
• Prepare and deliver required Medicare A cut letters and maintain documentation.
• Complete all PASRR paperwork and ensure compliance with state Medicaid requirements.
• Upload and maintain PASRR Level II documentation in PCC, ensuring follow-through on recommendations.

• The social worker must meet the following qualifications:
  • Minimum of a Bachelor’s degree in social work; OR
  • Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
• Strong leadership and communication skills with the ability to build positive relationships with residents, families, and staff.
• Prior long-term care or healthcare social work experience preferred.
• Knowledge of long-term care regulations, reimbursement systems, and quality improvement programs.
• Commitment to resident-centered care and continuous quality improvement.
