Qualifications:

Required:

* BSN degree

* Graduate of an accredited school of nursing required

* A minimum of 3 years acute care experience

Preferred:

* Organ transplant experience

* Knowledge of standard of care treatment for organ transplant

* Certified Clinical Transplant Nurse (CCTN) Certification preferred

Certifications and Licenses

Required:

* Active NJ RN license or compact RN license with NJ endorsement

* BLS from the American Heart Association

* ACLS from the American Heart Association

Scheduling Requirements:

* Day Shift, 8:30a - 4:30p (variable and not guaranteed)

* Full-Time, 37.5 hours

* Monday - Friday, position requires on call /and weekend and holiday hours

Essential Functions:

* The RN performs patient assessment, facilitates coordination of services and care, including: patient assessment, patient, family, staff and community education programs, and documentation.

* Performs direct patient care duties and triage patient phone calls.

* Participates in intra and interdisciplinary programs and committees, attend organ selection committee meetings, quality assurance performance improvement (QAPI) review meetings, departmental education meetings, and participate in hospital education to staff as warranted, which may include annual competency training and/or initial orientation.

* Edit, and maintain the electronic medical records and regulatory electronic data submission elements. Compile, calculate, and analyze statistical, medical, and operational data to report to government, regulatory, and medical organizations. Ensure the accuracy and completeness of required regulatory care plans and organ sharing forms (if indicated).

* Track quality data indicators and provide-on-time monthly reporting for department QA and for Intermacs.

Other Duties:

* Provide coordination and patient management to patients being seen on an outpatient and inpatient basis,

* Work closely with transplant team, including physicians, NPs, Social Workers, Dieticians, and Transplant Assistants to ensure high quality of patient care and patient satisfaction,

* Performs and documents patient assessments and medication reconcilations in patients with end stage disease management, pre-transplant & post-transplant that are referred to the office,(both outpatients & inpatients),
