Position DescriptionRegistered Nurse case manager in the Division of Environmental Health. The RN will visit clients in their homes to provide nurse case management services on a regular and emergency basis, depending on the child's Blood Lead Level. RN may be re‑assigned to perform other related duties within the Department of Health as needed.
Key Responsibilities- Perform home visits
- Prepare health assessments
- Prepare comprehensive health care plans
- Provide health counseling and teaching in the patient's primary language on lead poisoning prevention
- Determine whether the child has a primary care pediatrician or a health care facility for primary care
- Follow up in NJIIS to determine the child's immunization status
- Refer child to the immunization clinic as needed
- Monitor blood lead retesting and results, making referrals as needed
- Arrange lead screening for siblings and other children in the household aged six months to 72 months
- Assess need for emergency relocation funding and collaborate with appropriate agencies
- Assist family in obtaining cleaning supplies
- Learn to utilize Electronic Health Record systems including Welligent
- Close cases and discharge when all case management components are met
Licenses:
- Current and valid registration as a Professional Registered Nurse in New Jersey
- Valid driver's license (NJ) and personal vehicle to perform essential duties
- Knowledge of nursing principles, standards, and methods
- Knowledge of immunization and diagnostic procedures
- Knowledge of medical and laboratory procedures, tests, and analyses
- Ability to analyze, comprehend, and interpret nursing principles and case management practices
- Ability to establish and maintain cooperative working relationships with coworkers and stakeholders
- Ability to provide nursing care, first aid treatments, and perform laboratory duties (venipuncture)
- Ability to observe and report patient symptoms and conditions
- Ability to prepare comprehensive nurse case manager reports
