, a Modivcare Home care company in NJ is looking for a Per Diem Clinical supervisor
. The RN field nursing supervisor is responsible to ensure quality and safe delivery of home health Care services such as supervising Certified Home Health Aides in a patient’s home, evaluating the patient’s plan of care and assessing their physical and Psychosocial needs.

Schedule

1–2 days per week, daytime hours, covering Morris County and Bergen County. Visits will be scheduled with the patient’s aide present as required.

ntaf Compensation
  • Regular Visit, 2-Month Reassessment, and Supervision Visit: $45 per visit
  • Reassessment Visit: $55 per visit
  • Initial RN Assessment: $60 per visit

Mileage Reimbursement: Provided at the current federal mileage rate.

You will
  • Perform on-site supervisory and teaching visits to evaluatecaregivers
  • Provide clinical orientation to caregivers on assigned home care cases.
  • Participate in the training of new nurses and in-services for field staff.
  • Perform initial nursing assessments
  • Counsel patient/caregiver in meeting nursing and related needs.
  • Assign and coordinate services provided.
  • Assist physician in developing a plan of treatment/care and make necessary revisions based upon patient’s condition/needs.
  • Prepare clinical progress notes ensuring any changes in the patient’s condition is reported
  • Administer medication and treatments as prescribed and initiate appropriate preventative and rehabilitative nursing procedures.
  • Monitor clinical records for complete and accurate documentation.

  • Registered Nurse (RN)
  • 1 year of RN experience
  • CPR certification
  • 1 year of home care experience preferred
  • Bilingual (Spanish) Preferred
