This role is for an Advanced Practitioner supporting nephrology patients at a central New Jersey location.
Responsibilities- Work closely with physicians to manage patients with kidney disease, including CKD and dialysis patients.
- Perform patient assessments, develop care plans, and educate patients, ensuring compliance with regulations.
- Provide clinical services such as clinic support, hospital rounding, and on-call coverage, primarily Monday through Friday with occasional weekends.
- Require a Bachelor's degree, board certification (NCCPA or AANPCB), valid NJ license, DEA, and at least 2 years of clinical experience.
- Preferred experience includes chronic kidney disease management and private practice.
- Health insurance, 401(k), paid time off, and competitive salary ($109K-$125K).
- Must be able to travel within various nephrology settings and work in person.
