Community Health Workers use their unique position as trained community members to provide trust-based information and focused education, help clients cope with and overcome fear, guide clients through the healthcare system, and help them plan to manage the logistics of caring for themselves in the context of a complicated life. CHWs are role model peers who share experiences and community membership and are trained to provide services that increase retention and medication adherence/viral suppression.
Common CHW tasks include reminder phone calls, phone check-ins to ascertain reasons for missed appointments, and appointment accompaniment (e.g., assistance with lapsed health insurance coverage, medical care, legal services, social services, and SSI-related appointments). Peers can also perform community outreach and home visits as needed.
Education and Work Experience
- Commitment:
Passion about working to improve the health and quality of life of people living with HIV/AIDS. - Knowledge:
Demonstrated knowledge of HIV/AIDS and HIV medical care services in the region, with an interest and ability to expand knowledge through training. Knowledge of regional community resources/services is important. Ability to assess clients for needs related to treatment education, risk reduction and prevention. - Experience:
Demonstrated experience with issues related to HIV care and treatment. Familiarity with issues that can impact health, such as homelessness, incarceration, domestic violence, and/or substance abuse, is helpful. - Skills:
Strong communication skills; ability to conduct sensitive, empathetic interviews and conversations that respect client dignity and diversity. Ability to engage staff and clients in a supportive and empowering way, manage conflict, work effectively in a team, maintain client records, extract relevant demographic and disease information, and adhere to privacy and confidentiality protocols. Basic computer skills are required. - Personal qualities:
Energy, intelligence, diplomacy, sound judgment, the ability to work independently during less supervision, and the interpersonal skills to work effectively with program staff and others in the agency; resourcefulness and good judgment and decision making. - Education:
A high school diploma or GED is required. - Licenses: A valid driver’s license is required.
Pay Transparency
Saint Michaels Medical Center offers competitive compensation and a comprehensive benefits package. The Total Rewards package includes paid time off, a 401K retirement plan, medical, dental, and vision coverage, tuition reimbursement, and many more voluntary benefit options. A reasonable compensation estimate for this role, including wages, benefits, and other forms of compensation, is $22.28 to $55.00. The exact starting compensation will be determined at the time of hire, considering factors such as skillset, applicable experience, education, credentials, and licensure.
