The Certified Case Counselor II – MCM will be responsible for providing direct case management services to the patients of the Peter Ho Memorial Center. Responsibilities include client‑centered services linking clients with health care, psychosocial and other services, completing intakes, psychosocial assessments, developing individualized client service plans, advocating for client retention, monitoring referrals, re‑evaluating patients’ needs and discharge, treatment adherence counseling, and proper disposition of cases.

The role requires documentation in EPIC, NJDOH system (Care Ware), and Ryan White Grant reporting systems (CAHMPS) and involves collaboration with multidisciplinary teams and consults on HIV disease management. #LI‑EP1

• Minimum of two years’ experience in case management, discharge planning, or a related health‑care field.
• Master’s degree required; a Master’s in Social Work or another health‑related field and LSW licensure preferred.
• Ryan White Case Management Certificate obtained within 6 months of hire preferred.
• Basic to intermediate computer skills:
  Microsoft Office (Word, Excel, PowerPoint, email).
• Completion of MCM training within 3 months of hire and ongoing current certification.

The estimated wage range for this role is $16.50 to $29.07 per hour.
