Overview
GENERAL NATURE OF POSITION:
Provides case management to assist mental health, substance abuse and dual diagnosis clients. Assists clients to access medical, educational, social, prevocational, and rehabilitative or other needed community services for eligible individuals. Provides crisis intervention services for clients in various situations. Works with an interdisciplinary treatment team to assist the clients in their recovery and management of chronic mental illness, substance use, chronic health disorders and compliance with court requirements, etc.
In the spirit of the program, assist the client in accomplishing his/her goals.
- Provide a safe and welcome environment for people to visit.
- Answer phone calls and inquiries concerning the program.
- Maintain strict confidentiality guidelines regarding all clients, conversations and referrals.
- Manage an intensive caseload of 15-20 client files.
- Perform intake interviews and document data on all residents that reside at said site.
- Coordinate services with Clinical Director, Counselor (Interns), team members, primary care and specialist physicians, legal representatives/guardians, Departments of Mental Health and Alcohol and Drugs staff, probation, parole, pre-trial services, departments of citizen services and other community services/organizations to assure the individual’s access to services and the service delivery system.
- Assist clients in obtaining housing, financial assistance, health care and other community supports.
- Assist clients in completing applications for services, assistance, , etc.
- Provide outreach activities to identify potential clients who have been referred through Drug Court, SAI, Detox Centers, etc.
- Assist in developing materials and provide rehabilitation workshops on wellness, health, education, family relationships, relapse prevention, etc. for clients.
- Participate in a team environment to provide 24/7 client support.
- Follow respective documentation standards and document all activities according to program requirements.
- Record services in respective systems, maintain client files and complete monthly activity reports.
- Performs related work, as required.
- Must be willing to work weekends and/or holidays as needed.
Required and Preferred
Education and Experience:
- High School diploma.
- Ability to work with diverse populations.
- Good oral and written English skills.
- If an individual is in recovery, five-six years of sustained recovery is required.
- Computer skills:
Microsoft Office and web-based applications. - Experience with criminal justice system is a plus.
- Ability to lift 35 pounds
- Occasionally required to stand and walk
- Must successfully pass a background check prior to hire
