Beaumond House Hospice Care delivers personalised, high-quality end‑of‑life care across hospice, home and community settings. Patients and families remain at the centre of everything we do. All staff are expected to uphold dignity, safety, confidentiality and the highest standards of physical, psychological and spiritual care.

This role sits within the End‑of‑Life Care Together Alliance and operates across organisational boundaries to deliver safe, coordinated and person‑centred care for people in North Nottinghamshire.

The post is based in Beaumond House Hospice, Newark, with additional duties in community settings across North Nottinghamshire.

• Plan, coordinate, deliver and evaluate evidence‑based, person‑centred palliative care for patients, families and carers.
• Support the effective delivery of the Hospice at Home service within an allocated case‑load, working closely with the multidisciplinary team and partner agencies.
• Contribute to staff and service development, support admission avoidance initiatives and facilitate timely discharge to enable people to remain at home in accordance with their preferences and care plans.
• Provide 24/7 support, offering friendly ear, general information and signposting patients, relatives and carers to appropriate resources and services.
• Maintain accurate records and advance care planning in the person's preferred place of care.

Beaumond House Hospice supports patients and their loved ones to live as fully as possible across North Nottinghamshire, providing free care and comfort when every moment matters. The charity manages four in‑patient beds, day therapy and Hospice at Home care.
