This role sits within the End-of-Life Care Together Alliance and operates across organisational boundaries to deliversafe, coordinated, and person-centred care for people in North Nottinghamshire.

Main duties of the jobTo work within the End-of-Life Care Together Hub to provide coordinated,compassionate support to patients in the last year of life, including those intheir final days. The role focuses on ensuring patients, carers and professionals can access the right services at the right time, supportingadmission avoidance and timely discharge planning in line with individual
Advance Care Plans. Through effective care coordination, communication,administration and data management, the Care Navigator supports the work of clinical teams across the End-of-Life Care Together Alliance, while upholding the values, standards and ethos of Beaumond House Hospice Care.

Beaumond House Hospice Care is a registered charity. We provide four In-patient beds, Day Therapy, and Hospice at Home Care. Hospice at Home supports people with palliative and end of life care needs, often in the last weeks and months of life. Our nurses and health care assistants provide assessment, personal care, emotional support, advice, and advance care planning in the persons preferred place of care.
We are contactable 24 hours a day, 7 days per week, to provide a friendly ear, general information or to signpost patients, relatives and carers to appropriate resources and services.

Job responsibilitiesMain Duties
Care Navigation and Patient Support
- Act as a first point of contact for patients, carers and professionals contacting the End-of-Life Care Together Hub.
- Provide clear information about services and support options and help people navigate these.
- Support patients and families to understand and follow their Advance Care Plan and preferred place of care.
- Offer ongoing practical and emotional support, recognising changing needs and escalating to clinical staff when required.
- Work in partnership with primary and secondary care colleagues, members of the End-of-Life Care Together Alliance, the Beaumond House team and other support roles.
Referral Management and MDT Coordination
- Receive and process incoming referrals for palliative and end of life care in line with agreed protocols.
- Check referral information for completeness and seek further information where needed.
- Triage and direct referrals to the appropriate clinician or service within the Alliance.
- Arrange, attend and minute End of Life Care Together Hub MDT meetings.
- Prepare MDT agendas and caseload lists so all relevant health and social care professionals can attend and contribute.
- Record agreed actions from MDTs and follow up on these within agreed time frames.
Information Management and Administration
- Record all relevant contacts and interventions in electronic systems, including Systm One, e-Health Scope, Nervecentre and other agreed systems.
