Case Manager; Bronx & Brooklyn, NY

Compensation: $25-$27 per hour (based on experience)

In this role, you will support clients by connecting them with essential services, coordinating resources, and helping them achieve stability and long-term success. • Provide case management services to a diverse caseload of clients
• Conduct assessments, develop service plans, and track progress
• Connect clients to appropriate programs, resources, and community supports
• Maintain detailed documentation and case notes in accordance with agency and regulatory standards
• Collaborate with internal teams and external partners to ensure coordinated care
• Participate in team meetings, trainings, and outreach activities as needed

• Bachelor’s degree or at least 2 years of relevant case management or social services experience
• Strong communication, organizational, and documentation skills
• Ability to work with diverse populations in a fast-paced environment
• Familiarity with social service systems in NYC is a plus
