- Conduct recruitment efforts/activities and actively monitor the waiting list of eligible individuals/families to ensure accurate placement.
- Complete intake, enrollment, and ongoing assessments.
- Facilitate interdisciplinary collaboration among providers, the enrollee, family, caregivers, and all available supports.
- Ensure families are aware of all support services and entitlements, provide recommendations and referrals to relevant community resources, and advocate as necessary by serving as liaison between families and other programs.
- Conduct office, home, and community visits to assess family needs and service delivery.
- Perform outreach to program candidates within client populations in the community.
- Assist in coordination of services for special needs, providing necessary support to families who have children with disabilities.
- Monitor and maintain compliance with all Federal, State, and Local regulations.
- Monitor compliance using data management systems.
- Bachelor’s degree in Human Services or a related field.
- At least 2 years of experience in Human Services, Psychology, or a related field that included coursework in the principles of social work, child development, counselling, or psychology.
- Ability to plan and carry out assignments independently.
- Ability to prioritize, adhere to timelines, and multi‑task.
- Home visits are required.
