Position Overview

We are currently seeking a Transitional Counselor. The Transitional Counselor provides high-quality re-entry services to people returning from NYC jails. This includes assessment, service planning, referrals, case management, group facilitation, and other services as needed. The Transitional Counselor may go into jail facilities or reentry hotels or do other fieldwork. This position maintains progress records and provides written reports and statistics to funding sources and other regulatory agencies.

Salary

$27.80 - $29.18 hourly

Essential Duties
  • Offer outreach to participants prior to and following discharge to engage them in community-based program services
  • Provide discharge planning and engagement services inside of NYC jail facilities on Rikers Island or community-based jails
  • Complete a full psychosocial needs assessment
  • Develop individual service plans with the participant, identifying specific services and providers for each identified need
  • Source referrals and applications to obtain identifying documents, benefits and other entitlements, housing, medical care, behavioral health care, employment, education, family services, and services to address other needs that arrive
  • Provide ongoing case management, contacting participants at least once per week in the first 30 days and at least 2 times per month thereafter
  • Document all contacts and services as well as assessment, intake, service planning, and referrals in ECM promptly, within 24 hours of service provision
  • Engage in employment retention services as part of wrap-around case management services to employed participants
  • Maintain fidelity to evidence-based curricula and cognitive-behavioral interventions
  • Write and submit court letters indicating the progress of participants in these services as needed
  • Attend and participate in interdepartmental, subcontractor, and cross-functional meetings to increase awareness of services as needed
  • Facilitate group services/activities (i.e., Job Readiness, Financial Literacy, Anger Management, Parenting, Relationships, Substance Abuse Relapse Prevention, Creative Writing, etc.) as needed.
  • Complete admin tasks such as maintaining files, making photocopies, edits, and faxing documents as needed
  • Attend required training
  • In-person attendance is required to collaborate with co-workers
  • Perform other duties as assigned
Minimum Qualifications
  • Associate’s Degree in Criminal Justice, Social Work, Public Administration, or Human Services;
    In lieu of a degree, high school diploma or equivalent with two additional years of case management and group facilitation experience in the human services field
  • Two years of direct care experience in an individual and group setting
  • Two years of case management and group facilitation experience required
  • Experience working in correctional facilities and re-entry settings preferred
Key Competencies
  • Ability to travel locally
  • Excellent interpersonal and problem-resolution skills are necessary
  • Proficiency in Google and Microsoft Suite
  • Ability to work effectively under tight deadlines, pay strong attention to detail, and time management skills
  • Ability to work in teams and collaborate effectively with people in different functions
  • Ability to take the initiative and drive for results
  • Ability to communicate with all levels of the organization and build strong relationships
  • Bilingual Spanish speaking individuals are a plus.
