The Social Worker is part of a human service team that develops and executes service and care plans for residents, providing a supportive and empowering environment that fosters stabilization, independence, and self‑reliance. The Social Worker assesses resident needs, provides direct services, bridges residents to external providers, and promotes healthy behaviors.
Pay: $70,000 annually
FLSA: Exempt. Not eligible for overtime.
Schedule: Monday – Friday 10am–6pm or Tuesday – Saturday 10am–6pm (Sunday and Monday off).
Location: Midtown Manhattan, NY
- Service and Care Coordination
- Ensure residents receive quality and appropriate services per organizational principles.
- Conduct comprehensive biopsychosocial assessments to identify immediate and long‑term needs (healthcare, food, childcare, employment, immigration, trauma, etc.).
- Collaborate with case managers to design service plans that address identified needs.
- Coordinate workshops and activities that promote self‑care, cultural adjustment, and language acquisition.
- Work with external providers to access preventive, remedial health and mental health services.
- Meet with residents weekly to review progress, modify plans, and conduct room inspections for hygiene and housekeeping.
- Assist with discharge plans and ensure referrals to community organizations.
- Intervention Services
- Develop and facilitate workshops with external providers; monitor engagement and impact.
- Provide crisis intervention and de‑escalation to ensure safety of residents, staff, and visitors.
- Follow up after priority incidents to bridge residents to services.
- General Duties
- Document interactions and communications in resident records.
