Position: Housing Case Manager
Location: New York

Pay or Shift Range: $23 USD to $26 USD per hour. Final offers based on skill set, experience, location, qualifications and other job-related factors.

Compensation Range

$23/hr - $26/hr commensurate with experience

Overview

The Housing Case Manager (HCM) is responsible for case management and supportive care coordination for assigned clients and their households. The STARS 6 Young Adult family program is a scattered‑site supportive housing program for young families. The HCM works closely with their clients in the homes, community, and office to ensure housing stability through independent living skills, financial management, and health and wellness.

Responsibilities
  • Effectively manage assigned caseload, including engagement, referrals, concrete needs, and client chart compliance.
  • Provide referrals and follow‑ups for primary care, substance use, nutrition, clothing, mental health, financial management, and dental services.
  • Identify and schedule case conferences with internal and external providers.
  • Coordinate with departments such as ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job‑Training.
  • Maintain chart compliance with eICare and TMS (Maven Mental Health System) per guidelines.
  • Assist clients in obtaining and maintaining benefits such as SNAP, SSI, Medicaid, and rental subsidies.
  • Participate in interdisciplinary teams supporting the resident by coordinating with internal and external providers.
  • Conduct and document service provision in eICare as outlined by contract deliverables.
  • Help new clients move into apartments and transition from homelessness to stable housing.
  • Collaborate with clients to develop strength‑based individualized goals and provide feedback during scheduled engagement.
  • Work with Child Development Specialists to ensure children receive needed services.
  • Discuss financial management with clients, follow‑up on rent obligations, subsidy maintenance, utility payments, and nutritional needs.
  • Accompany clients to community appointments as needed.
  • Attend bi‑weekly supervision meetings and be prepared.
  • Co‑facilitate Parenting Journey groups and other thematic group activities.
  • Travel via mass transit with clients or independently for client needs.
  • Provide crisis intervention and mediation when necessary.
  • Cover for other Housing Case Managers when needed.
  • Participate in annual training such as Motivational Interviewing I & II, Smoking Cessation, and Trauma‑Informed Care.
  • Assist clients with budgeting, rent arrears counseling, ADL training, and orientation on program structure.
  • Complete required reports and paperwork by deadlines.
  • Demonstrate excellent customer service with residents, staff, vendors, and stakeholders.
  • Advocate and take direct action to end homelessness and HIV/AIDS crises.
Requirements
  • BSW or related degree; MSW preferred.
  • Experience with and commitment to working with young adults and marginalized communities, including young children.
  • Communication skills, both verbal and written, for all levels of management.
  • Proficiency with Microsoft Outlook and Office (Word, Excel, PowerPoint).
  • Cultural competency, especially regarding LGBTQI+ communities.
  • Dependable, flexible, and maintain client confidentiality.
  • Strong internal and external customer service and interpersonal skills.
  • Ability to work under pressure and solve problems with colleagues.
