Are you passionate about supporting families during challenging times? We are hiring Case Aides for two specialized programs that make a meaningful difference in the lives of children and caregivers.
Our Special Medical Preventive Services program supports families where a parent is living with a chronic or terminal illness or where a child has special medical needs. The goal is to empower families, promote safety, and prepare for long-term stability, including permanency planning when necessary.
Our Family Treatment/Rehabilitation (FT/R) Program serves high-risk families throughout the Bronx, addressing the safety and wellbeing of children when a caretaker's or child's substance use or mental health disorder places them at risk of foster care placement.
As a Case Aide
, you will provide essential support to Case Planners and families, helping with service coordination, documentation, and engagement to ensure children grow up in safe, nurturing environments.
- Supplement case planner contacts in accordance with ACS minimum casework contact standards as directed.
- Assist with outreach and engagement efforts.
- Provide or arrange case coverage during workers' vacations or other leave.
- Continually assess family engagement, behavior changes, and in-home functioning.
- Advocate for and escort families to service sites, modeling appropriate behaviors and assessing learning transfer.
- Maintain collateral contact with service providers and request progress reports; share information with the case planning team.
- Actively participate in monthly case supervisions and clinical diagnostic team meetings.
- Aid in the development of comprehensive assessments.
- Ensure compliance with ACS training and program learning mandates.
- Participate in ACS and agency meetings, including FTCs, as directed.
- Ensure all written documentation and PROMIS contacts are entered accurately and promptly.
- Assist with maintaining physical case records.
- Perform additional duties as required to meet department/agency needs.
- High School Diploma required;
Associate's Degree preferred. - Some knowledge of the Child Welfare System and experience counseling children and families.
- Basic computer skills. >
- Flexibility to meet changing needs and demands.
- Fluency in Spanish and English preferred.
- Ability to travel throughout the Bronx and to other CMCS locations.
Strong organization and time management skills.
