The Discharge Specialist is responsible for managing a caseload of approximately 12–20 youth who are on trial discharge. In addition to overseeing their own cases, the Discharge Specialist serves as a supportive resource to Case Planners, helping facilitate a smooth and sustainable transition toward permanency. This role focuses on partnering with families to strengthen effective parenting practices, reduce behavioral and emotional challenges, and promote positive youth development.
The Discharge Specialist will also assist families in identifying and addressing stressors that may impact stability—whether related to navigating community resources (such as housing or public assistance) or challenges associated with the reunification process.
Maintain accurate and timely case documentation in Connections and other agency systems, including filing, progress notes, and all required records. Engage and assess families to identify strengths, needs, risks, and cultural factors to ensure services are responsive, respectful, and culturally appropriate. Conduct regular home visits and collateral contacts in accordance with ACS and agency policies to assess safety, family functioning, and compliance with the service plan.
Education& Experience
BA and two (2) years of Child Welfare is required. Experience preferred. Bilingual Spanish speaking required.
Physical RequirementsWhile performing the duties for this job, the employee is regularly required to communicate clearly and effectively. The employee must occasionally lift and /or move up to 25 pounds.
