Social Worker / Client Care Coordinator
OverviewVolunteers of America – Greater New York (VOA‑GNY) is an anti‑poverty organization dedicated to ending homelessness in the New York area by 2050. We operate over 60 programs across New York City, Northern New Jersey, and Westchester County, providing emergency, transitional, and permanent housing. Our services support families experiencing homelessness, survivors of domestic violence, people living with HIV/AIDS, veterans, youth aging out of foster care, adults with behavioral health or substance use needs, and others, including a special education preschool program.
The Social Worker / Client Care Coordinator will enhance existing services to include a thorough assessment of families’ long‑term goals at intake and the development of a service plan that incorporates small, achievable tasks leading to permanent housing, enrollment, and/or referrals to community resources. The role is based at Legacy Family Residence, a transitional family shelter located in the Bronx, where residents learn essential life‑skills.

Minimum Qualifications- Master’s Degree in Social Work with the expectation that an LMSW is acquired within six months of hire, or a Master’s Degree in Mental Health Counseling with an LMHC license.
- Knowledge of adult, child, and adolescent development, emotional/behavioral health, mental health, family dynamics, parent‑child relationships, and diagnostic classifications.
- Experience using strengths‑based, solution‑focused, and family‑centered practice.
- Experience working with diverse client populations.
- Knowledge of child welfare policies, regulations, safety, permanency, and well‑being.
- Manage a caseload.
- Complete psychosocial assessments for all families in shelter and develop individualized service plans focusing on recovery and wellness.
- Prepare assessment summaries and determine the type of intervention needed for each family.
- Maintain service documentation, including client progress notes, observations, interactions, and incident reports when necessary.
- Provide on‑call crisis intervention support.
- Participate in individual service coordination activities, including vocational and educational services and support for recreational and socialization activities.
- Develop and maintain community linkages.
- Refer families to appropriate community providers to address their needs.
