New Alternatives for Children, Inc. (NAC) is an award‑winning not‑for‑profit agency located in Midtown Manhattan dedicated to serving children and families with medical complexity, chronic health conditions, significant disabilities, and behavioral challenges. NAC is currently seeking a Case Planner to join its Specialized Family Foster Care Department.
Position Details- Title: Case Planner (Bachelor & Master Level)
- Type: Full Time
- Location: 825 7th Avenue, New York, NY 10019
- Pay Range: Bachelor Level: $50,000 – $55,000. Additional compensation for bilingual skills. Master Level: $60,000 – $65,000. Additional compensation for bilingual skills and licenses (LMSW, LMHC, etc.)
- Department: Specialized Family Foster Care
- Provides casework and clinical services to children and families using individual, group, and family modalities; conducts assessments and coordinates services for a caseload of 11‑12 foster children as recommended by NAC’s multidisciplinary treatment team.
- Conducts home and community‑based visits to families; supports parent‑child visits both in person and virtually.
- Works collaboratively with psychiatrists, psychologists, pediatricians, nurses, education specialists, and fellow clinicians in a dynamic, strength‑based interdisciplinary team model.
- Prepares for and participates in Family Team Conferences and NYC Family Court proceedings.
- Completes ACS and NAC paperwork timely and ensures documentation conforms to Agency, City, and State regulatory requirements, including progress notes, court reports, and the Family Assessment and Service Plan (FASP).
- Makes adequate use of individual and group supervision, on‑site/virtual clinical trainings, and mental health in‑service.
- Bachelor Level: BSW, Bachelors in psychology, human services, or related field.
- Master Level: Master’s degree in social work, mental health counseling, human services, psychology, public administration, or related field.
- Prior experience in foster care, child welfare, and/or health care preferred.
- Excellent written and verbal communication and organizational skills.
- Strong engagement skills.
- Proficiency in Spanish is preferred (and includes a salary differential).
