Job OverviewUnder the supervision of the Senior Housing Supervisor, the Housing Case Manager coordinates care for resident services, providing education, support, and accountability to ensure residents receive the services they need and the program meets contractual and regulatory obligations.
Responsibilities- Meet regularly with the Senior Housing Supervisor for file documentation review.
- Participate in agency orientation covering program operations, policy, and goals.
- Facilitate integration of residents into the community and use community-based services whenever possible.
- Maintain accurate case file records and reports per funding source/program requirements.
- Document assessments, service plan goals, reviews, utilization reviews, hospitalizations, discharge planning, progress notes, and income in accordance with agency policy and procedure.
- Submit resident files to the Senior Housing Supervisor within 28 days of admission for audit and review.
- Renew file documents with expiration dates such as ROI, consents, physician authorizations, housing quality inspection packets, and income verification.
- Ensure licensed apartments meet recertification standards; report needed maintenance or supplies.
- Ensure resident fees related to rent and phone bills are paid on time and assist with delinquency issues as needed.
- Assist residents in complying with treatment/service providers, ensuring no lapse in service or funding.
- Advocate and mediate disputes involving landlords, tenants, providers, employment, and funding issues.
- Assist residents transitioning to more or less restrictive levels of care, ensuring continuity with providers and funding streams.
- Serve as a resource to ensure residents have access to services and that confidentiality rights are preserved.
- Conduct routine housing inspections, identify residents in need of help, communicate concerns to the Senior Housing Supervisor, and work with residents to maintain their units or access additional services.
- Attend staff meetings, treatment team/provider meetings, and staff development/training seminars per agency policy.
- Complete other duties as assigned.
- Maintain clear background checks as defined by regulations, the Justice Center SEL, Fingerprinting, and NYS Central Registry.
- Understand and perform mandated reporting for child abuse or neglect as required by law.
- Bachelor’s degree in human services or related field preferred.
- Five years’ experience in the Human Services field and three years’ experience in a related supervisory position; a combination of education and experience may be substituted.
- Minimum two years’ experience in Human Services or related field.
- Valid NYS Driver’s license with a clean driving record acceptable to the agency.
- Reliable, registered, and insured vehicle if required by the role.
- Background checks meeting agency and regulatory standards.
- Licensed residence and housing quality knowledge.
- Demonstrate ethical business practices in conformance with state and federal laws.
- Excellent verbal and written communication skills.
- Ability to multi‑task and prioritize duties.
- Willingness to foster agency, department, and programwide cooperation through positive communication.
- Maintain absolute confidentiality of consumer, employee, and billing records.
- Proficiency with PCs and Microsoft applications.
- Data analysis and problem‑resolution skills.
