Under supervision of the Director of Social Service, the Case Manager provides case management, housing support to clients, and monthly reporting to ensure compliance with program parameters.
Base Pay Range$40,000.00/yr – $45,000.00/yr
Key Essential Functions Client Advocacy- Conduct needs assessments and case management for assigned families.
- Conduct intakes, home visits, and apartment inspections.
- Hold bi‑weekly interviews with clients.
- Maintain up‑to‑date files including client history and profile.
- Provide information and referrals, arrange services, advocate and intervene with government agencies, offer short‑term counseling, and assist in developing service plans aimed at restoring family independence.
- Coordinate workshops, seminars and other activities for the general client population.
- Perform related duties as assigned.
- Make recommendations to improve service levels.
- Meet with the Head of Household and begin filling out Section 8, NYCHA and/or Homeless Housing applications, completing documentation in a timely manner.
- Meet with the Head of Household every two weeks to determine housing status and immediate needs.
- Contact landlord/broker to schedule apartment visits.
- Follow up with landlord/broker to ensure Section 8 package or related documents were submitted.
- Schedule Head of Household for apartment inspection with the housing site.
- Follow‑up meeting with resident for lease signing.
- Follow‑up meeting after lease signing to schedule resident for move‑out.
- Contact Public Assistance agency to request rent checks.
- Provide case management and onsite crisis intervention.
- Complete required service plans.
- Educate participants on good housekeeping, home‑safety practices, budgeting, and landlord/tenant relations as needed.
- Maintain appropriate documentation of all tenant contacts.
- Document monthly progress notes in the participants’ chart.
- Conduct home visits to evaluate compliance with program rules and assess need for referral to supportive services.
- Report any alleged child neglect or abuse to the ACS agency or authorities.
- BA degree in a relevant field (Master’s preferred)
- Minimum of 2 years of Case Management experience
- Experience working with homeless populations
- Housing knowledge
- Familiarity with community resources
- Excellent oral and written communication skills
