$24.47 - $30.22 / hour
Position SummaryThe Solution Based Casework (SBC) Case Planners assist parents/caregivers in addressing barriers to their children’s health, safety and well‑being so families can stabilize, strengthen and reduce future involvement with the child welfare system. Primary responsibilities include making safety‑focused assessments, developing intervention plans, and helping families activate change.
Responsibilities- Understand and promote the organization’s Mission, Vision, and Core Values.
- Maintain an average caseload of 12 families with at least 2 contacts per month, one in the home.
- Complete all required documentation (SBC model and ACS).
- Provide services including family and individual therapy, advocacy (public assistance, child support, housing, school), educational monitoring, parenting support and general case management.
- Receive training and certification in the Solution Based Casework Model.
- Collaborate with and refer to appropriate service providers for case coordination.
- Maintain up‑to‑date progress notes, complete Family Assessment and Service Plans, input contact data into PROMIS and CONNX, and obtain required documents from ACS.
- Participate in weekly individual supervision, monthly group supervision, and other professional development activities.
- Schedule and participate in Family Team Conferences.
- Attend staff meetings and division events as required.
- Complete required training hours through the Workforce Institute (ACS).
- Perform additional duties as assigned.
- Bachelor’s degree in Social Work (B.S.W.) or equivalent.
- Interest in learning family therapy theories and interventions.
- Commitment to a strengths‑based framework.
- Self‑starter who intervenes actively.
- Excellent oral and written communication.
- Ability to manage many tasks and respond to crises.
- Strong engagement skills with children, teens, parents or families.
- Bilingual in Spanish/English preferred.
