Salary: $50,000-$55,000/yr
Position SummaryThe Case Planner works collaboratively with therapists, education specialists, and social workers to manage a caseload of approximately 10 children with varying mental health needs. The role focuses on providing intensive casework and field services to support permanency goals, while also conducting outreach, coordinating Family Team Conferences, maintaining case records, utilizing the CONNECTIONS system, and participating in case consultations.
Essential Duties- Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
- Work with a dynamic team of professionals including nurses and social workers in our foster care program for children with special medical needs.
- Provide intensive casework services and fieldwork for children and their birth and foster families with the goal of providing permanency as quickly as possible.
- With medical staff, team-train foster parents in specialized childcare skills.
- Conduct intensive outreach to connect children and families with agency and community treatment resources.
- Maintain written case records.
- Work in partnership with RN assigned to cases;
Collaboration includes conducting home visits whenever possible. - Perform other duties as assigned.
- Bachelor's degree in social work or related field required.
- Foster care experience preferred.
- Bilingual preferred.
- Valid Driver License.
- Excellent verbal, writing, and organizational skills.
- Flexibility, and ability to work under pressure and meet deadlines.
- Computer literacy.
- Team Player - Works well as a member of a group
- Detail Oriented - Capable of carrying out a given task with all details necessary to get the task done well
- Dedicated - Devoted to a task or purpose with loyalty or integrity
Bachelors of Social Work (required)
ExperienceBilingual (Spanish/English) preferred. (preferred)
Foster care experience preferred. (preferred)
