Location: Village of Pleasantville

Work in partnership with case planning and interdisciplinary treatment team to assure a safe and permanent outcome for every child. Provide case management for youth therapeutic services to children and their families.

Essential Generic

Job Functions:

Client Contact to Support Permanency
  • Participate in family team conferences.
  • Participate in family and/or sibling visits as requested.
  • Meet with youth and families in their home and community while in care and post-discharge.
  • Crisis intervention and support as needed
Teamwork to Facilitate Permanency
  • Assist in developing comprehensive, intensive, short-term plans to stabilize the youth and familial situation to avoid a potential disruption of placement
  • Facilitate and support communication between the youth, family, any other outside stakeholders, and the JCCA team
  • Collaborate with team members and Local Department of Social Services to generate and support individualized case management, referrals for treatment and discharge plans.
Document of Permanency Services
  • Maintain progress notes in NYS EHR Connections and complete monthly contact grid.
  • Development of a formal written discharge support plan
OTHER TASKS AND REQUIREMENTS:
  • Represent agency in Court Hearings
  • Refer and follow-up with community resources
  • Available for evening and weekend work and participation in special events
  • Complete FASPS, PHR's and court reports as assigned
Knowledge, Education and

Skills Required:
  • Bachelor's Degree in Social Work, Human Services, or related field
  • Demonstrate ability to collaborate with client and teams that are culturally and professionally diverse.
  • Computer Literacy (specifically Microsoft Word, Excel).
  • Ability to navigate NYS Connections system.
  • Solid organizational and follow-up skills
  • Excellent telephone and interpersonal skills
  • Excellent writing skills.
  • Ability to work independently
Knowledge, Education and Skills Preferred:
  • Fast Learner/Adapts Quickly to new tasks
