Job OverviewSamaritan Daytop Village operates transitional housing programs for single adults and families experiencing homelessness across all five boroughs of New York City. The Evening Case Manager works directly with clients to conduct interviews, evaluate their needs, and collaboratively develop Independent Living Plans. The role involves identifying community resources, coordinating referrals to healthcare, education, legal support, and assisting clients in relocating to permanent housing.
The manager maintains detailed documentation, monitors progress, and advocates to remove barriers to stability and self‑sufficiency.
- Build professional relationships with clients and maintain confidentiality
- Review client eligibility documents and explore non‑shelter alternatives
- Conduct intake interviews and psycho‑social assessments
- Develop and update Independent Living Plans with short‑ and long‑term goals
- Refer clients to permanent housing and community‑based services
- Support clients in overcoming barriers and advocating for themselves
- Track client progress and document updates through regular contact
- Follow up with clients and referral agencies to monitor service outcomes
- Provide data for required reports (weekly, monthly, quarterly, annual)
- Act as a liaison with outside organizations for education, health care, childcare, housing, and legal matters
- High School Diploma or equivalent required;
Associate or Bachelor’s Degree in Human Services, Social Work, or a related field strongly preferred - At least 1 year of case management or social services experience working with individuals experiencing homelessness, substance use, mental health challenges, or other complex needs
- Strong interpersonal skills and ability to build trust with diverse clients
- Clear and effective communication skills for interviews, documentation, and coordination with partners
- Ability to maintain accurate records and meet reporting deadlines
- Commitment to client confidentiality and professional ethics
- Familiarity with NYC housing, healthcare, legal, childcare, and educational resources
- Problem‑solving skills and a supportive approach to helping clients overcome challenges
- Organized and detail‑oriented, able to manage multiple responsibilities
- Culturally sensitive and responsive to the needs of individuals and families experiencing homelessness

This is a full‑time, non‑exempt position with an hourly rate of $23.53, annualized at $42,840 for 35 hours per week.
