We are seeking a full‑time Registered Nurse to join our Adult Medicine department at Urban Health Plan located at the El Nuevo San Juan Health Center in the Bronx, NY. As an RN, you will provide comprehensive nursing support and patient care in an outpatient primary care setting.

• Bachelor's or Associate's Degree in Nursing is required.
• NYS Registered Professional Nurse required.
• All experience levels welcome to apply; whether this is your first nursing role or you are a seasoned nurse.
• Bilingual English/Spanish is strongly preferred.
• Excellent communication skills, compassion, empathy, and team‑driven attributes are essential.
• Knowledge and training in electronic health records and Microsoft Office.

• Triage patients and assist patients with their needs based on acuity.
• Provide quality nursing care and medication administration; administer immunizations/vaccines to specified patients.
• Monitor patient reactions and side‑effects.
• Assist care team with pre‑visit planning for high‑risk patients.
• Ensure implementation of plans of care.
• Act as a role‑model and source of nursing guidance to medical assistants that are part of the team.
• Assist with prepping patients as needed and provide patient education.
• Assist providers during examinations and treatments.
• Ensure proper sterilization procedures using germicides, sterilizers, or autoclave.
• Ensure departmental adherence to clinical policies and procedures.
• Maintain health records by documenting patient contact and updating patient profiles.
• Implement standing orders.
• Check equipment, logs, and medical kits; assist with ordering medical supplies, medications, and vaccines.

Minimum $82,560 – Maximum $98,560 per annum, contingent on degree and experience level.

Onsite, Bronx, NY. Monday to Friday 8:00 a.m.–5:00 p.m./9:00 a.m.–6:00 p.m.; rotating Saturday shifts with a day off M‑F.
