Family Nurse Practitioner

Premier Health Care (Premier), a member of the YAI Network, is recognized by the US Surgeon General as a national model for the provision of health care, and by the National Council on Disabilities for raising the national standards of health care services for children and adults with developmental and intellectual disabilities. Premier has also earned national certification as a Level 3 Patient-Centered Medical Home by the National Committee on Quality Assurance (NCQA).

Most appointments will be conducted through Tele Health with a few exceptions that will require onsite treatment.

Responsibilities include but are not limited to:

• Patient-Centered Quality and Safety
• Provide clinical oversight for treatment, including COVID-19 patient testing and Tele Health platforms
• Educate patients on maintaining proper health
• Evaluate, make recommendations, co-manage and treat patients' medical needs for safe and high quality treatment
• Document all patient care within an electronic health record (IDX / Next Gen)
• Focuses consistently on the patient
• Creates a warm and welcoming environment
• Manages multiple demands and needs of clinic operations, patients in the waiting room, while maintaining focus and high quality care on the patient in the exam room

• 2-3+ years of Nurse Practitioner experience preferred
• Providers will be assigned a primary site, however, are expected to travel to and staff other locations within a manager-defined area as needed by the business

• Master's of Family Nurse Practitioner, or similar program
• NYS of Employment license to practice in the Advanced Practice Nurse

• Current National Board Certification
• Credentialed through Medicaid and/or Medicare

• $125/hr.
