Nurse Practitioner (NP) – Telehealth

$150,000–$175,000 annually (based on experience)

We are seeking a full-time Nurse Practitioner (NP) to provide Telehealth services for our Primary Health Care Clinic in Brooklyn. This role focuses exclusively on virtual patient visits and is ideal for an NP who is confident in independent clinical decision-making, efficient documentation, and delivering high-quality care in a remote setting while being part of a collaborative clinical team.

This is NOT a remote role. The position will work in office.

• Active New York State Nurse Practitioner license — required
• National NP certification (FNP, Adult, or related specialty)
• Experience in telehealth, primary care, urgent care, or outpatient medicine preferred
• Strong diagnostic, assessment, and virtual patient communication skills
• Comfortable managing patient volume in a telehealth environment
• DEA registration (or ability to obtain)
• Proficient with EMR systems and virtual care platforms

• Conduct comprehensive telehealth consultations via video and phone
• Diagnose and manage acute and chronic conditions
• Order labs, imaging, and referrals as appropriate
• Prescribe medications in accordance with NY State regulations
• Provide patient education, follow-up care, and care coordination
• Maintain accurate and timely documentation in the EMR
• Collaborate with in-clinic providers, nursing staff, and administration
