We are seeking a dedicated Field Nurse to join our team at Wellbound Certified Home Health Agency. In this role, you will provide compassionate and proactive nursing care to our patients in the assisted living facility, ensuring they receive the highest standard of health services tailored to their individual needs.

• Deliver high-quality nursing care to patients in their assisted living facility, focusing on both clinical and functional needs.

• Conduct thorough assessments of patient health conditions and develop individualized care plans.

• Administer medications and treatments as prescribed by physicians.

• Educate patients and families on health management and self-care techniques.

• Collaborate with interdisciplinary teams to coordinate comprehensive patient care.

• Monitor patient progress and adjust care plans as necessary based on evolving health conditions.

• Insulin injection

• Wound care

• Valid nursing license in the state of New York.

• Experience in home health care or similar settings preferred.

• Strong communication skills to effectively interact with patients and families.

• Ability to work independently and manage time efficiently while traveling between patient homes.

• Experience working with children and understanding their unique healthcare needs is highly desirable.
