POSITION OVERVIEWA Licensed Practical Nurse (LPN) is responsible for nursing care and medical procedures within the scope of practice, assisting in the integration of physical and behavioral health in program. The LPN reports to the RN or Nursing Leadership.
KEY ESSENTIAL FUNCTIONS- In line with doctor’s orders, administer prescribed and/or routine medical treatments while ensuring internal policies regarding administration and documentation.
- Assist with scheduling appointments and obtaining medical documentation prior to the client’s appointments.
- Function as the liaison between external medical health care providers, schools, other community organizations and the facility with regards to the client’s medical needs.
- Provide general nursing care within the scope of LPN practice, including taking and recording client’s vital signs, administering prescribed medication and noting time and amount on client’s chart.
- Observe clients and report adverse reactions to medication or treatment to supervisor.
- Phlebotomy and specimen preparation to be sent to laboratory.
- Inventory and requisition supplies.
- Respond to medical emergencies immediately upon notification; perform triage to stabilize client and provide needed treatment within scope of practice; follow up for complete recovery.
- Perform charting and other documentation as required by internal practice and external regulatory bodies.
- Complete other tasks within the scope of an LPN NYS license as requested by RN or Nursing leadership.
- Translate research and other knowledge to improve practice processes and outcomes.
- Communicate practice knowledge effectively, orally and in writing.
- Participate in agency quality improvement projects to ensure best client outcomes.
- Create a climate of patient-centered care, including confidentiality, privacy, comfort, emotional support, mutual trust, and respect.
- Must hold an active LPN nursing New York State license.
- Must maintain active CPR certification.
- Excellent oral, written, communication and organizational skills.
- Excellent aural and written communication and collaboration skills.
- Excellent organizational skills and data management capabilities.
- The candidate should be able to read documents for analytical purposes such as computer information, software, computer graphic design programs, etc.
- Limited applications of manual dexterity and hand-eye coordination.
- Ability to walk, sit, stand, and bend as necessary to assess and provide care to clients effectively in various settings.
- Must be capable of responding swiftly to emergencies, which may involve moving quickly to the scene, performing CPR, and utilizing appropriate emergency protocols.
- Proficient in lifting, transferring, and repositioning clients safely when needed, ensuring the comfort and safety of both clients and staff.
- Maintain the physical stamina to perform nursing duties throughout the shift, including prolonged periods of standing and walking, while remaining attentive to patient needs.
- Ability to navigate a variety of environments, including tight spaces, while maintaining a focus on patient care and safety.
