The Pediatric Registered Nurse plans, organizes, and directs home care services. The role requires experience in pediatric nursing from birth through late teens, emphasizing community health education.
Responsibilities- Complete comprehensive assessments of patients and families to determine home care needs and provide a physical assessment and history.
- Regularly re‑evaluate patient nursing needs.
- Initiate the plan of care and make necessary revisions as patient status changes.
- Develop the plan of care in collaboration with the patient’s physician, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative actions.
- Initiate appropriate preventive and rehabilitative procedures; administer medications and treatments as prescribed.
- Provide counseling to patients and families.
- Offer health care instructions per assessment and plan.
- Identify discharge planning needs and implement them prior to discharge.
- Apply high‑tech nursing skills such as indwelling catheter insertion/removal, nephrostomy care, ostomy care, catheter drainage, infusion services, G‑tube care, and negative‑pressure wound therapy.
- Maintain infection control procedures and safety precautions.
- Apply the nursing process within the scope of practice, demonstrating professional theory and practice.
- Observe ethical and legal guidelines for patient confidentiality.
- Assess patient needs and physical status each skilled visit through health data assessment and patient interview; re‑evaluate changes and inform the physician of any updates.
- Prepare clinical notes and update the primary physician at least every sixty days; submit all documentation per policies.
- Communicate with physicians regarding patient needs and changes in condition; obtain orders as required.
- Coordinate care with other health‑related persons.
- Participate in on‑call duties per policy.
- Ensure arrangements for equipment and necessary supplies and services are available.
- Instruct, supervise, and evaluate home health aide/personal care aide care every fourteen days or as required by law.
- Instruct, supervise, and evaluate LPN/LVN care at least every thirty days.
- Coordinate care among home, community, and tertiary pediatric specialists.
- Bachelor's degree in Nursing from an accredited college or university.
- Active and unrestricted nursing license.
- Pediatric care experience in acute and/or home care environments.
- Valid driver’s license.
- Certification in Basic Life Support for Health Care Professionals.
- Minimum one year of recent acute or home care experience, preferably in an institutional setting.
- Excellent observation, verbal and written communication, problem‑solving, basic math, and competency checklist nursing skills.
- Basic computer proficiency for communication and documentation.
