Rockaway Home Care is hiring Per Diem Registered Nurses (RNs) and Outpatient Nurses to support our growing patient population throughout Manhattan, The Bronx, Brooklyn, and Staten Island. • Conduct in-home or community-based assessments

• Perform head-to-toe evaluations including medication and diagnosis review

• Complete aide supervisions and provide clinical guidance

• Develop and update personalized Plans of Care (POC)

• Provide orientation to aides based on the POC

• Accurately document findings and maintain compliance with healthcare regulations

• Current New York State RN license

• Ability to travel throughout the 5 boroughs, Nassau, and Suffolk

• Medical Professional Liability insurance

• Comfortable using Gmail and Microsoft Word/Excel.

• Bilingual in Spanish/English

• Familiarity with Medicaid, Medicare, or MLTC plans

• Experience in outpatient nursing, home care, or community health

• Proficiency in the
