Job Description

Performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Responsibilities
  • Assess the patient’s physiologic health status, conduct patient interview, explain policies and procedures, review chart, assess gastrointestinal, cardiovascular, respiratory, renal and neurological health status, determine mobility, sensory deficits, prostheses use, and skin condition, assess level of pain and pain management, communicate and document the patient’s physiologic health status and plan of care.
  • Assess the patient’s psychosocial health status, elicit perception of medical/nursing care, determine coping mechanism, knowledge level, ability to comprehend, identify cultural/ethnic requirements to reduce anxiety, communicate and document psychological status and care plan, provide support group, counseling services, social services.
  • Identify patient outcomes: develop criteria for measurement, identify actual/potential patient problems, identify patient’s need for teaching, develop patient outcome statements and individualized goals.
  • Formulate the plan of care and establish priorities: identify care activities, coordinate cost‑effective use of supplies, equipment and medication, document plan of care and collaborate with physicians and other health team members.
  • Implement the plan of care: maintain vigilance over patient care, exercise professional skills, reassess patients as needed, administer prescribed treatments, use equipment, provide emotional support, apply scientific principles safely and efficiently, observe and record patient conditions, document interventions, act rapidly during emergencies, communicate with family, plan discharge, provide discharge instructions.
  • Evaluate care provided for patient outcome: measure effectiveness, perform variance analysis, design, implement, and evaluate systems to improve care, document outcomes.
  • Collaborate with other care team members: collaborate with physician, interpret and implement treatment regimen, assist physician during treatments, keep patient‑care manager updated, delegate assignments to support staff and ensure timely completion.
  • Perform grade I‑IV decubitus care: preventive skin care measures, apply dressings with aseptic technique, use warm & cold compresses, perform irrigations and other skin‑care procedures as necessary.
  • Administer medications correctly and safely: identify medication action, dosage, side effects, meet standards on medication administration exams, prepare local solutions and IV drips, administer and document medication, educate patients on drug interactions.
  • Participate in patient and family education: explain conditions, communicate assessment data, recognize and utilize teaching opportunities, provide early discharge planning, evaluate effectiveness of teaching.
  • Perform related duties as required.
  • PHYSICAL DEMANDS:

    • Constant – standing, walking, stooping, bending, stretching, lifting, pulling, pushing and turning.
    • Frequent – lifts objects from 10 lbs to 150 lbs. Pushes objects up to 200 lbs.
    • Occasional – sitting. Lifts with assisted objects greater than 150 lbs and carries objects weighing up to 50 lbs.
