As a Case Manager, you’ll be more than a caregiver—you’ll be a critical thinker and trusted advocate, delivering high-quality, patient-centered care where it matters most: n a team that believes in supporting independence, dignity, and healing—while giving you the flexibility to build your own schedule. Typical hours are 8:00 AM – 4:30 PM, Monday through Friday, with on-call and weekend rotations (approximately one weekend per month).
Flexibility with accountability is key: you’ll plan your daily schedule based on your patient caseload.

PositionSummary
Responsible for the case management and provision of quality home nursing services to patients within the agency’s philosophy, policy, and Standards of Community Health Nursing Practice.
Education, License & CertGraduation from an approved School of Nursing, Bachelor’s Degree in Nursing preferred.
ExperienceOne‑year previous nursing experience with Medical/Surgical Nursing required. Prior Public Health/Community Health experience preferred.
Essential FunctionsThe pay rate for this position is $81,120.00 - $ per year.
